(Appointment Only) We accept Health Insurance plan on PREMERA In-Network, we also see clients who have Personal Auto Injury Protection Claims (PIP). Our team of licensed and certified massage therapists offers expertise in various techniques, including medical therapy, sports massage, and deep tissue massage. We are a PREMERA in-network provider and accept clients with personal auto injury protection (PIP) claims. Important Notice: Appointments can only be scheduled after all required documentation has been submitted. Please kindly provide below documents for Permera insurance massage process: 1. Copy front and back sides of your insurance card and ID (driver license) 2. Doctor referral including two ICD-10 pain codes. 3. Cancellation Policy

1. An additional $3 transaction fee will be charged for each transaction made using a Credit/Debit card or Apple/Google Pay. There is no additional fee for cash payments. 2. Each therapist has a separate tips account. To thank them for their efforts. Please tip them individually per transaction. (You may pay for all services at once, but please tip in cash for convenience.)
